The Recovery Continuum was founded by Courtney Taylor, BSN, RN, and Sheniqua Fortune, BSN, RN, CPAN, CAPA, two experienced perioperative nurses who recognized a critical gap between hospital discharge and recovery at home. After years of caring for post-surgical patients, we saw firsthand the challenges patients and families face once they leave the hospital. Our mission is to bridge that gap by providing skilled, compassionate, RN-led recovery support that promotes safer healing, reduces complications, and gives patients the confidence to recover comfortably at home. We are committed to extending continuity of care beyond discharge—because recovery doesn’t end when a patient leaves the hospital.

Why We Exist

The Discharge Gap Is a Patient Safety Crisis

Every year, millions of patients are discharged from hospitals with a stack of papers, a prescription, and a follow-up appointment weeks away. In between, they're on their own — navigating complex medication regimens, wound care protocols, and warning signs they don't know to look for.

As experienced perioperative and post-anesthesia nurses, we have witnessed firsthand the challenges patients and families face after leaving the hospital or surgery center. While patients may be medically stable for discharge, many still require guidance, support, education, and skilled nursing oversight during the critical days that follow surgery.

We created The Recovery Continuum to bridge the gap between hospital discharge and successful recovery at home. Our purpose is to provide professional, RN-led support that extends continuity of care beyond the healthcare setting, helping patients recover safely, comfortably, and confidently in the place they want to be most—home. Because healing deserves more than discharge instructions.

With more than a decade of experience in perioperative and post-anesthesia nursing, Courtney has cared for patients across a wide range of surgical specialties and recovery settings. Her passion for patient advocacy and continuity of care inspired the creation of The Recovery Continuum, where she helps patients transition safely and confidently from hospital discharge to recovery at home.

As a Certified Post Anesthesia Nurse (CPAN) and Certified Ambulatory Perianesthesia Nurse (CAPA), she brings exceptional clinical expertise and compassionate care to every patient recovery journey.
