Patients are often discharged medically stable but still face significant challenges during the first days of recovery. The Recovery Continuum provides RN-led post-operative support that extends continuity of care beyond discharge, helping patients recover safely at home while supporting the recovery plan you worked hard to create.

The Case for Partnership
Readmission rates, patient satisfaction scores, and complication rates are all tied directly to what happens in the weeks after a patient leaves your care. Yet most surgical practices have no visibility into that window.
The Recovery Continuum gives you that visibility — and the clinical infrastructure to act on it. We monitor your patients daily, document their recovery, and communicate proactively with your office so you're never caught off guard at a follow-up.
Our structured monitoring catches complications early — before they become ER visits. Referring providers see an average 38% reduction in 30-day readmissions.
Patients who feel supported after surgery rate their overall surgical experience higher. Our program consistently lifts HCAHPS and Press Ganey scores for referring providers.
Every follow-up appointment is preceded by a structured progress report — symptoms, wound status, medication compliance, and any concerns flagged during monitoring.
You hear about problems before your patient does. Our escalation protocol ensures your office is notified immediately when something warrants clinical attention.
Beyond Discharge
Comprehensive RN evaluation following discharge, including pain management, mobility, hydration, and recovery status.
Ongoing monitoring and support during the most vulnerable phase of recovery.
Reinforcing physician instructions and helping caregivers feel confident at home.
Serving as an extension of the recovery plan while keeping communication pathways open when concerns arise.

Provider Questions
Patients who are recovering from surgery, live alone, have limited caregiver support, are traveling for procedures, or would benefit from professional nursing oversight during the first hours and days following discharge.
Our experienced Registered Nurses reinforce discharge instructions, monitor recovery progress, support medication adherence, assist with wound and drain management, educate caregivers, and communicate concerns when appropriate.
Unlike non-clinical caregivers, Registered Nurses possess the assessment skills and clinical expertise necessary to recognize subtle changes in condition, identify potential complications early, and provide skilled recovery support during a critical phase of healing.
There is no cost to referring providers. Our services are billed directly to patients or their insurance. We handle all billing and authorization independently.
When our team identifies a concern that warrants physician attention, we contact your office immediately using the escalation protocol you specify at onboarding. We document all escalations with timestamps and clinical notes.
Help your patients recover with confidence through RN-led support that extends continuity of care beyond discharge.
